Please complete this form with payment of $180 per child, per session.  All forms and payment must be completed in full and returned before your child may start the program and in order to secure a spot in the program. Enrollment is on a first come, first serve basis.  No spots will be held for a child by phone or email. 

Enrollment Agreement 2025

1. I authorize Salt Pond Areas Bird Sanctuaries, Inc. (SPABS) its affiliates to have, use, publish and reproduce photographs of my child for its records or publications.
2. I recognize that there are inherent risks associated with Outdoor Discoveries activities and I grant permission for my child to participate in SPABS organized activities.
3. I grant permission for SPABS staff to arrange for appropriate emergency medical care for my child as necessary.  In the event of serious illness or injury requiring hospitalization, the staff will use all reasonable means to contact me.  I understand SPABS is not responsible for any and all charges incurred from emergency medical treatment.
4. I will have a responsible adult pick up my child at Bourne Farm at the designated time.  I have read and understand the drop-off/pick-up policies.
5. I understand that SPABS has financial obligations based on expected enrollment and administrative costs. If it is necessary to withdraw my child, requests for refunds of registration fees must be made within 30 days of enrollment in writing and No refunds will be made after the 30 days.  Refunds during the 30 day period will only be made if the vacated slot is filled by a child on the wait list.
