Thank you for enrolling your child in our program.

All forms and payment must be completed in full and returned before your child may start the program and in order to secure a spot in the program. Enrollment is on a first come, first serve basis.  No spots will be held for a child by phone or email.

Participant Information Form:

The Participant Information form must be filled out separately for each child. For example, if you are enrolling 3 children, you must complete this form 3 separate times.

Parent Enrollment Agreement Form:

You will need to fill this form out once, listing each child and the sessions they are participating in.
